ADMINISTRATIVE GUIDELINE TITLE: Student Mobile Device Checkout

I. Statement of Purpose
This guideline documents the procedures for credit students to check out mobile devices from the West Burlington and Keokuk campuses.
II. Users Governed
This guideline applies to SCC credit students and employees.
III. Student Check Out Process
- Students will be required to complete an online “Student Mobile Device Checkout” form available on Hawknet under “Useful Links”. There will be a separate checkout form for Keokuk and West Burlington students.
- Students will contact an SCC instructor or student advocate and ask them to confirm the need for a mobile device.
- SCC instructor or student advocate will complete “Mobile Device Confirmation” form available on Hawknet under “Useful Links”.
- Once steps “A”, “B” and “C” are completed above, SCC will contact the student
within three business days to schedule a time to pick up the device. If a mobile device is not available, SCC will contact the student to communicate you have been placed on a wait list. - Students will be required to sign and complete the following information before checking
out the mobile device:
- Date the device will be returned
- ID – Photo ID required (example: driver’s license)
- Sign to confirm responsibility for the condition and security of the device
- Students will need to login to the mobile device on the wireless network with their SCC email address and password before leaving campus. Students will not be able to login to the mobile device off campus if this step is missed.
IV. Lost, Damaged or Stolen Mobile Device
If the device is lost or damaged, the student is responsible for the cost to replace the device. If the device is stolen, the student will be responsible for working with SCC ITS staff to file a police report. A hold will be placed on the student’s account if the device is lost or damaged until the balance is paid.
